Healthcare systems in Nebraska often work in silos, leading to a lack of coordinated care for seniors who require various services, such as medical, social, and mental health support. Many seniors suffer from multiple chronic conditions, complicating their healthcare needs further and impacting their quality of life. Fractured communication between providers can result in suboptimal care coordination, sometimes leading to harmful medication interactions and recurrence of health issues. According to the Nebraska Department of Health and Human Services, over 34% of older adults involved in a study expressed frustration in accessing the healthcare they needed due to these gaps in service delivery.
To address these systemic issues, funding is allocated for implementing collaborative care teams specifically targeting the needs of seniors in Nebraska.
